Transitioning Teens and Young Adults with Asthma from Pediatrics to the Adult Model of Care, is an important one for all of us caring for adolescents and young adults with asthma. Providing our patients and their families with the knowledge and skills to not only participate in asthma goal setting and self management but to promote and support their independence in gradually assuming personal responsibility for their care as they approach the transition to adult care.

• Discuss the concept and practice of transitioning as it applies to individuals with asthma
